A Clinicostatistical Study of Laryngeal Diseases Which were Performed Laryngeal Microsurgery


Abstract
In 1960, Scalco et al performed the first laryngomicrousugery using Lynch's suspension laryngoscope. Kleinsasser reported a new technique of layngomicrosurgery with self retaining suspension laryngoscope in 1968.
The techniques of laryngomicroscurgery has advanced due to development of microscope, laser and methods of anesthesia.
We made a statistic analysis of 53 cases(50 patients) of the suspension laryngoscopic microsurgery in department of Otolaryngology-Head and Neck surgery from August, 1985 to November, 1990.
@ES The results were as follows:
@EN 1) Sex distribution was male 1 : female 1.03.
2) Age distribution showed 24 cases(45.3%) in 4th decade, 12 cases (22.6%) in 3rd decade, 7 cases(13.2%) in 6th decade and 4 cases(7.5%) in 5th decade in order.
3) Chief complaints were horseness 47 cases(88.6%), lump sensation 2 cases(3.8%) and known laryngeal trauma 2 cases(3.8%) in order.
4) Preoperative diagnostic impressions were as follows:
vocal polyp 24 cases(45.3%), laryngeal nodule 14 cases(26.4%), papilloma 5 cases(9.3%), cyst 3 cases(5.7%) and granuloma 3 cases(5.7%) in order.
5) Histopathlogic results were as follows:
laryngeal nodule and polyp 38 cases(71.7%), papilloma 6 cases(11.2%) and granulation 3 cases(5.7%) in order.
6) The time between the onset of disease and the operation were as follows:
17 case(32.1%) from 1 months 3 month, 12 cases(22.6%) form 3 months to 6 months and 5 cases (9.4%) from 9months to 12 months in order.
7) The distibution of involved sites were as follows:
single 4.4 : both 1, and 1/3 of vocal cord 41 cases(77.4%).
8) The methods of anesthesia were as follows: 31 cases(58.5% under intravenous anesthesia with high frequency jet ventilation, 22 cases(41.5%) under general intubation anesthesia.
9) The main instruments for operation were as follows:
40 cases(75.5%) with biting cupped forcep and 8 cases(15.0%).
